29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: a. On or about 11/6/2019, throughout the distribution center, where employees are trained and expected to fight incipient level fires the employer failed to place fire extinguishers in locations that would not be blocked with stored materials in locations including but not limited to E 034 and E 020. b. On or about 11/6/2019, throughout the distribution center, where employees are trained and expected to fight incipient level fires the employer failed to identify fire extinguisher placement in locations including but not limited to E 034, E 020, F 037, F 013.

29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly: On or about 11/6/2019, throughout the distribution center, where employees are trained and expected to fight incipient level fires the employer failed to inspect the portable fire extinguishers on a monthly basis since July 24, 2019. During the walk-around inspection there was a discharged extinguisher identified at location F 13 since September 2019, there were blocked extinguishers at locations including but not limited to E 034 and E 020, and none of the extinguishers mounted on poles in the center of the distribution center had location identifying markings.
