1926.21(b)(2) Respondent failed to instruct employees in the recognition and avoidance of unsafe conditions resulting from working on top of an elevator cab. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location: 1158 McDonald Ave. Brooklyn NY On or about: 06 NOV 2019 a) Employees were working on the top of an elevator cab with a unprotected side and exposed to a fall hazard of approximately 15 feet.
