29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: On or about October 28, 2019, an employee un-jamming the jaw-separating machine was exposed to a moving chain and sprocket and the moving parts were not adequately guarded, allowing the employee to reach inside the nip point resulting in an amputation.
