29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) At Mill 1, on or about December 17, 2019 - Machine guarding was not provided to protect the Weld Mill Operator from hazards created from in-going nip points of rollers in the roll-forming section of the Oto 383 pipe mill. b) At Mill 7, on or about December 17, 2019 - Machine guarding was not provided to protect the Weld Mill Operator from hazards created from in-going nip points of rollers in the roll-forming section of the T & H pipe mill. c) At Mill 16, on or about December 17, 2019 - Machine guarding was not provided to protect the Weld Mill Operator from hazards created from in-going nip points of rollers in the roll-forming section of the Oto Mill pipe mill.
