29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by an employee working from an aerial lift:  At Hotel: On or about 10/30/2019 and at times prior, the employer exposed an employee to fall hazards of approximately 16 feet from ground level, in that an employee did not attach a lanyard to the harness while working from an aerial lift.

29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  At Hotel: On or about 10/30/2019 and at times prior, the employer exposed an employee to fall hazards of approximately 16 feet from ground level, in that an employee climbed the rails of  an aerial lift to reach a higher level to install forms on a column.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  At Hotel: On or about 10/30/2019 and at times prior, the employer exposed employees to fall hazards of approximately 20 feet, in that fall protection was not utilized by employees while  performing rebar work at the edge of the 2nd floor deck.
