29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. 6068 Buford Hwy. Norcross, GA (Shop Area) - The employees (shop mechanics) were exposed to sudden start-up of the gas powered ride-on core aerators equipment. The employer did not have machine specific written procedures for tagging out the ignition/energy source to the core aerator machines to safe guard employees performing services such as but not limited too, servicing of the aerator belt drive and and/or maintenance on the equipment, on or about 11/07/19.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a. 6068 Buford Hwy. Norcross, GA (Shop Area)- The employer failed to notify the Atlanta-East OSHA office of an accident resulting in a amputation injury, within 24 hours of the accident. The employee suffered an amputation to his left hand middle finger November 1, 2019 and was reported to the OSHA Area Office until November 5, 2019.
