29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  Q & C Construction LLC, on or about November 11th, 2019, at the location of 4144 Secor Rd., Toledo, OH, 43623:  The employer did not maintain accident prevention programs to ensure that employees  were provided with instruction to protect themselves from workplace hazards.  Deficiencies in jobsite safety included employees exposed to fall hazards of approximately 17 feet without appropriate fall protection and an employee exposed to falling object hazards without appropriate head protection.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) Q & C Construction LLC, on or about November 11th, 2019, at the location of 4144 Secor Rd., Toledo, OH, 43623:  The employer did not ensure that employees tasked with working below roofing activities were protected from the hazards of falling objects through the use of a protective helmet or hardhat.  Potential falling objects included tools, roofing materials, and other employees.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   (a) Q & C Construction LLC, on or about November 11th, 2019, at the location of 4144 Secor Rd., Toledo, OH, 43623:  The employer did not ensure that employees performing roofing work were protected from falls on all sides on the roof of a commercial building.  Employees were exposed to fall hazards of approximately 17 feet to the ground below.   Q & C Construction LLC was previously cited as Richard Seger Construction for a violation of an equivalent standard to this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1366194, citation number 1, item number 2, and was affirmed as a final order on 01/23/2019, with respect to a workplace located at 4142 Douglas Rd., Toledo, OH, 43613.

Q & C Construction LLC was previously cited as Richard Seger Construction for a violation of  this occupational safety and health standard, 29 CFR 1926.503(b)(1), which was contained in OSHA inspection number 1366194, citation number 2, item number 1, and was affirmed as a final order on 01/23/2019, with respect to a workplace located at 4142 Douglas Rd., Toledo, OH, 43613.
