29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about November 04, 2019 and at times prior thereto, the employee observed standing 40-feet above ground on the mid-rail of the extended Wilkie 72R extended sign crane aerial basket, had not been properly trained to recognize the fall hazard.
