29 CFR 1910.178(l)(4)(ii)(B): Refresher training and evaluation was not provided to the operator when the operator was involved in an accident or near-miss incident. a) Loading Area - On or about November 4, 2019, a forklift operator was involved in an accident when the load he was carrying struck a coworker on the left shoulder. On November 7, 2019, the forklift operator was observed operating the forklift to load a trailer and had not yet received refresher forklift operator training.
