29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 11/04/19, on the roofing jobsite at a 2-story house, at 90 Prospect Avenue, Buffalo, NY: One employee was engaged in roofing work on the right side, towards the back of the house. The employee was exposed to a fall of approx. 17 ft. because fall protection was not being utilized. The pitch of the roof was approx. 8 in 12. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 11/04/19, on the roofing jobsite at a 2-story house, at 90 Prospect Avenue, Buffalo, NY: An extension ladder was set up on the ground in front of the house and was used by an employee to access the upper roof. The siderails of that extension ladder were approx. 4 inches above the roof landing. NO ABATEMENT CERTIFICATION REQUIRED
