29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) 4285 Kozy Korner Road - On or about November 4, 2019, employees are utilizing an aluminum extension ladder to access an elevated working area that is not placed at least three feet above the upper landing surface.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    a) 4285 Kozy Korner Road - On or about November 5, 2019, an employee is performing housekeeping work on the ground, while debris was being thrown to grade by employees removing roofing material, without utilizing any means of head protection.     GMH Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1313009, citation number 1 item number 1 and was affirmed as a final order on May 29, 2018, with respect to a workplace located at 5000 Big Island Drive, Jacksonville, FL.    GMH Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1315297 citation number 1, item number 1 and was affirmed as a final order on September 18, 2018 with respect to a workplace located at 2634 NE 43rd Rdoak Hill Plantation, Ocala, FL.
