29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: On or about November 7, 2019 at the worksite located at 1515 Community Way in Gainesville, GA employer failed to ensure that each personal fall arrest systems (PFAS) were inspected before use. An employee was exposed to fall hazards of approximately 20 feet when the employee's PFAS did not allow the leg straps to be fastened due to the inability to adjust the straps to fit the employee, while utilizing an aerial lift to reach the working level.
