29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: On or about November 16, 2019, Employees performed residential siding construction on a ladder jack scaffold at heights of greater than ten feet without fall protection. This condition exposed employees to the hazard of falling to the ground below.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On November 6th, the employer reported the hospitalization of an employee greater than 24 hours after it occurred. This condition delayed the timely investigation of the workplace accident.
