On or around November 3, 2019 and at times prior thereto, the employer at Dow Chemical Company Glycol #2 facility in Plaquemine, Louisiana did not ensure the check valve on the vapor feed line from D-410 (Knockout Drum) to T-410 (Acid Scrubber) remained functional to prevent 5% sulfuric acid solution from back flowing into D-410, which contained liquid ethylene oxide.
