29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:   a) On or about 11/8/19, at the second and third floor balcony area of the site, East Amherst, NY. Employee, installing sidings and trims from a 6-foot step ladder, positioned himself at 4-rung up the ladder over the height of guardrails, 90-degree away from the step ladder and was exposed to fall to the ground; 2 or 3 stories high. Employee was supposed to always position the step ladder in front of him and he turned 90-degree away from the ladder.   NO ABATEMENT CERTIFICATION REQUIRED
