29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a)  King Specialties Inc.- Packing area/ Box making area:   The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing, maintenance, unjamming, lubrication and cleaning operations on machinery, they made sure that the  electric and pneumatic energy sources were rendered inoperative.  Machinery included but was not limited to: SWF Box maker machine Model # SWF1T3 S/N 4934, Grupo Giro Bagging Machine, Model #GB/UB, S/N 2632805. Occurred on or about 11/12/2019.

29 CFR 1910.178(g)(2):  Battery charging apparatus for industrial trucks located in battery changing and charging installation was not protected from damage by trucks:  a) King Specialties Inc.- shipping and Receiving area:  The facility did not protect the battery charging apparatuses from damage.  The charging cables on a Helmar Inc. Lead Acid battery charger S/N TM1829 had damaged cords with exposed copper conduit and the charger outer casing was damaged, observed on or about 11/12/2019.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by ingoing nip points and rotating parts:  a) King Specialties Inc. - Packing area and box making area:  The SWF box maker machine #4934, Model SWF1T had an insufficient guard.  Employees are exposed to in going nip points from two wheels that grab the carboard to fabricate boxes, occurred on or about 10/12/2019  b) King Specialties Inc. - Packing area and box making area:  Employees are exposed to pinch points from the moving rolls at the end of the conveyor #6 while they working near and walking around it, occurred on or about 10/12/2019

Occurred on or about 11/12/2019.
