29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard  assessment had been performed through a written certification which included the  requirements as outlined in 29 CFR 1910.132(d)(2):   (a) Sunhouse Colorado LLC, 4640 Ironton St., Denver, CO 80239: On or before November,6, 2019,  the employer did not ensure through written verification that a workplace personal protective equipment (PPE) hazard assessment had been performed.     Abatement Note: The employer shall verify that the required workplace assessment has been performed through a written certification that shall include the following:       1)   Identifies the workplace evaluated;       2)   The person certifying that the evaluation has been performed;       3)   The date(s) that the hazard assessment was performed; and       4)   Which identifies the document as a certification of hazard assessment.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  (a) Sunhouse Colorado LLC, 4640 Ironton St., Denver, CO 80239: On or before November,6, 2019, the employer did not provide medical evaluations for employees required to wear tight fitting full face respirators prior to the use of the respirators.   Note:  The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.
