On or about November 13, 2019, employees were using a partially planked scaffold to access the trusses where they were laying decking, exposing themselves to a 10.7-foot fall hazard.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:    a.  On or about November 123 2019, employees used a 12ft. wide scaffold plank to access/egress from the garage floor to the second floor of the house where they were laying plywood, exposing employees to a 14-foot fall hazard.

On or about November 12, 2019, employees were working on top of the trusses without a fall protection system, exposed to a 16-foot fall hazard.
