29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) JR Gutters & Roofing Company, LLC at1009 Corona St. Denver, CO 80209: On or before 11/13/19 the employer did not protect employees form injury in that employees working on the 12/12 and 7/12 steep pitched roof of a two story single family home were not wearing fall protection. This condition exposed employees to fall hazards of 18 and 8 feet.

29 CFR 1926.503(a)(2): At this job site, the employer had not assured that each employee has been trained, as necessary, by a competent person qualified in fall hazards: (a) JR Gutters & Roofing Company, LLC at1009 Corona St. Denver, CO 80209: On or before 11/13/19 the employer did not ensure that employees received effective training in fall protection by a competent person in accordance with this standard. Employees were working while using their personal fall arrest systems improperly, thus increasing the hazard of their work space. This condition exposed employees to fall hazards.
