ACI Const. Co., Inc. jobsite located at 539 South Main Street, Findlay, OH: On or about November 6, 2019 the employer did not ensure employees working on the third floor of the MPLX building were trained on the following information: 1. The requirement to test for induced voltage and backfeed voltage on circuits prior to exposing themselves to potentially energized conductors. 2. The requirement to test for voltages between 50 and 100 volts on potentially live conductors prior to performing work.

29 CFR 1926.416(a)(1): The employer permitted an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee was protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means. a. ACI Const. Co., Inc. jobsite located at 539 South Main Street, Findlay, OH: The employer did not ensure employees performing electrical work on the third floor of the MPLX building de-energized the lighting circuit prior to performing the work. Employees de-energized the circuit prior to disconnecting the bank of lights but after the lights were disconnected from the main feed, they re-energized the circuit. On November 6, 2019 an employee was attaching the wires from a recently installed bank of lights to the energized feed wires inside of a junction box and suffered a severe electric shock.

29 CFR 1926.416(a)(3): Before work is begun the employer did not ascertain by inquiry or direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, is so located that the performance of the work may bring any person, tool, or machine into physical or electrical contact with the electric power circuit. The employer shall post and maintain proper warning signs where such a circuit exists. The employer shall advise employees of the location of such lines, the hazards involved, and the protective measures to be taken. a. ACI Const. Co., Inc. jobsite located at 539 South Main Street, Findlay, OH: On or about November 6, 2019 the employer did not ensure employees performing electrical work on the third floor of the MPLX building were properly testing de-energized circuits to determine inadvertent induced voltage, back feed voltage, or voltage between 50 and 100 volts was present. The employees were using Fluke voltage detection pens to determine if voltage was present and the pens were not rated below 100 volts.

Test voltages and the maximum intervals between tests shall be in accordance with Table E-4 and Table E-5. a. ACI Const. Co., Inc. jobsite located at 539 South Main Street, Findlay, OH: On or about November 13, 2019 the employer did not ensure insulted gloves used for protecting against electrical shock, were tested every 6 months. The gloves located in the maintenance department, available for employees to use were last tested in January 2015.
