29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): A) At the Lake Ellen Apartments, employees doing roofing work on a 4:12 pitch roof without using a fall protection system were exposed to a fall hazard of approximately 25 feet. On or about November 11, 2019. B) At the Lake Ellen Apartments, the roof nearly collapsed under the weight an employee removing roof shingles that was not using any fall protection equipment and exposed to a fall of approximately 25 feet. On or about November 11, 2019.

The side rails of the ladder did not extend 3 feet above the front porch roof and exposed the employees to a fall hazard of approximately 9 feet. On or about November 11, 2019.
