29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: B&B Machine and Grinding Services Corporation at 303 West Evans Street in Denver, CO 80223: On and before November 13, 2019, the employer did not ensure that the workplace was assessed to determine if hazards were present which would necessitate the use of personal protective equipment. Employees operate CNC machines, drill presses, abrasive wheel grinders, and other tools and equipment in the shop. This condition exposed employees to hazards associated with these tools and equipment. Abatement Note: The employer shall verify that the required workplace hazard assessment has been performed through a written certification that shall include the following: 1) The workplace evaluated; 2) The person certifying that the evaluation had been performed; 3) The dates of the hazard assessment; and, 4) The document identified as a certification of the hazard assessment.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: B&B Machine and Grinding Services Corporation at 303 West Evans Street in Denver, CO 80223: On and before November 13, 2019, the employer did not provide respirator users with the information contained in Appendix D. Employees should be provided with P-95 filtering facepiece respirators for protection from dust, mist, and other aerosols if they desire to use voluntary respiratory protection. Employees have been wearing N-95 type respirators which are not oil resistant. This lack of training for voluntary respirator use exposes employees to the hazard of not understanding the limitations of their respirators.

29 CFR 1910.141(g)(2): Employees were permitted to consume food or beverage in areas exposed to toxic materials B&B Machine and Grinding Services Corporation at 303 West Evans Street in Denver, CO 80223: On and before November 13, 2019, employees were allowed to consume food and beverages in the Blanchard Grinder area. This area is heavily contaminated with petroleum products and liquid waste containing metal particles from Blanchard grinder. This condition exposed employees to potential ingestion of hazardous chemicals.
