29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a. Entire facility - On or about November 14, 2019, the employer did not ensure that employees who were allowed to operate the Toyota (serial #42993) sit down, Yale (serial # E815N02377D) standup, and other powered industrial trucks on a daily basis were not trained and evaluated in accordance with paragraph (l). Thereby employees were exposed to struck-by and crushed-by hazards.
