29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    (1) prepare for shut down by recognizing energy hazards and its controls,  (2) turn off and shut down the machines,  (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4) affix lockout or tagout devices to each energy isolating device,  (5) render safe all potentially hazardous stored or residual energy,  (6) verify that isolation and de-energization of the machine or equipment have been accomplished.    Pigtail department No. 2720, Schafer Megomat wire processing machine asset No. 79-609: on or about November 7, 2019, operators assigned to setup, clean, and clear jams on energized equipment had their fingers exposed to point-of-operation pinch point during brass terminal strip feeding and cut-off terminals removal because the control of hazardous energy(lockout) was not done in the sequence as required by 29 CFR 1910.147(d)(1)-(6).    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.    Pigtail department, Schafer Megomat wire processing machine asset No. 79-609: on or about November 7, 2019, a work-related incident that resulted in an amputation was not reported timely to OSHA within twenty-four (24) hours, either in person to the OSHA Area Office that is nearest to the site of incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on the OSHA public web site at www.osha.gov    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
