The employer manufactured lifting device did not have required items such as, but not limited to; rated load, identification, and any applicable product safety labels.  b) Glacier Pre-Cast Concrete, Inc.: On November 12, 2019 and at times prior, employees were exposed to struck-by hazards while using an, in house fabricated below-the-hook device to strip and roll a three phase electric vault form. The employer manufactured lifting device did not have required load testing prior to use.
