29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors:    (a)  On or about 11/6/19 - In the shop, a safe working procedure for cleaning of a fuel tank had not been developed and was not utilized.  Employees were exposed to burn hazards due to the presence of diesel fuel vapors when cleaning activities are performed.

29 CFR 1910.133(a)(3):  The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses:    (a)  On or about 11/06/19 - Shop, an employee was exposed to metal dust while operating a portable tool during cleaning without adequate eye protection.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which described how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) would be met:    (a)  On or about 11/14/19 - at the site, the employer had not developed or implemented a hazard communication program for the site where employees have potential exposures to diesel fuels, welding gases, and machine oils. The program should address safety data sheet maintenance and availability, employee information and training, and information provided on labels.
