29 CFR 1910.134(e)(1):  The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace:     (a) On and before 11/15/2019, at facility, for the welder/fabricator wearing 3M half-face respirator with P100 cartridges while welding and cleaning a dusty machine.  The employee was not provided a medical evaluation prior to being required to wear the respirator.     R & J Sheet Metal Distributors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134(c)(2)(ii), which was contained in OSHA inspection number 1093542, citation number 1, item number 3 and was affirmed as a final order on 3/12/2016, with respect to a workplace located at 156 Orange St. Albany, NY 12210.

The employer did not implement storage procedures.  R & J Sheet Metal Distributors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134(c)(2)(i), which was contained in OSHA inspection number 1093542, citation number 1, item number 2 and was affirmed as a final order on 3/12/2016, with respect to a workplace located at 156 Orange St. Albany, NY 12210.

The employer did not train employee on respiratory protection. 29 CFR 1910.134(f)(2):  The employer did not ensure that employees using tight fitting face piece respirators pass an appropriate qualitative or quantitative fit test prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter.  (a) On and before 11/15/2019, at facility, for the welder/fabricator wearing 3M half-face respirator with P100 cartridges while welding and cleaning a dusty machine.  The employee was not fit tested, either qualitatively or quantitatively, prior to being required to use this respirator by the employer.
