29 CFR 1910.22(c): The employer did not provide, and/or did not ensure each employee uses, a safe means of access and egress to and from walking-working surfaces.  a) On 11/13/2019, in the Cheese filler line #8, employees don't have a safe way of accessing the step on the motorservo machine to load/remove the lids on the 5th and 6th row of the machine. The step is approximately 3 feet of the ground and employees have to climb on from the ground or step stool that is approximately a foot of the ground exposing employees to fall hazards.    ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   a) On or about 11/13/2019, in the packaging room, employee entered robot area in Line #6 & #4 without locking out the equipment to unjam a box and clean yogurt that had spilled over the sensor light. When employee cleaned the yogurt off the sensor, it activated the equipment and caught his middle finger causing a fingertip amputation.  b) On or about 11/13/2019, in the packaging room, when employees perform maintenance and servicing activities, such as but not limited to setting up the box maker, energy control procedures were not being utilized, exposing employees to unexpected start of moving parts inside the box maker.   ABATEMENT DOCUMENTATION REQUIRED
