(b) Plastics Department Hot knife #1655-2 - On or about 10/25/19, a rotating and reciprocating parts of a hot-knife machine were not guarded, exposing employees to amputation, caught-in, and struck-by hazards. Eagle Manufacturing Company was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1026227, Citation number 1, Item number 1 and was affirmed as a final order on April 1, 2015, with respect to a workplace located at 2400 Charles St, Wellsburg, WV.
