OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard:    a. On or About 11/08/19, in Growing Room #17, on the second level - Planks left in the rolled up position created opening on the elevated walking/working surface thus exposing employees harvesting mushrooms to fall hazards. An employee suffered serious injuries after falling through an opening on the walking/working surface.    Abatement Note: An acceptable means of abatement would be to fully plank the walking/working surfaces and/or ensure that a fall protection system (guard rails, covers, nets or personal fall arrest system) be in place for employees working from elevated walking/working surfaces.
