Employees used Accurate Companies Grill-O to clean ovens and ranges.  Grill-O was sprayed onto surfaces and then wiped off.  Grill-O was a corrosive liquid.  Eye protection was not worn by employees.  The employer did not ensure that an emergency eyewash was provided within the work area.  This condition exposed employees to a chemical hazard.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:    (a)  29th Street ALF LLC, dba Meadowview of Greeley, at 5300 W 29th Street, Greeley, CO: On or about 9/11/19, when outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures.  The facility contained two elevators.  The elevators had multiple energy sources, including electrical and hydraulic power.  29th Street ALF LLC had not developed or documented procedures for de-energizing either elevator.  An outside employer performed maintenance of the two elevators on 9/11/19.  29th Street ALF LLC did not inform the outside employer of any procedures for the de-energization and control of the electrical power and hydraulic power for the elevators.  This condition exposed outside employees to an uncontrolled energy hazard.

Employees disposed contaminated sharps.  Employees performed tasks, including housekeeping and responding to spills of blood or Other Potentially Infectious Materials (OPIM).  The employer had not reviewed and/or updated the Exposure Control Plan annually.  This condition exposed employees to a bloodborne pathogens hazard.

29 CFR 1910.1030(d)(4)(iii)(A)(2)(i): During use, containers for contaminated sharps were not easily accessible to personnel or located as close as was feasible to the immediate area where sharps were used or could be reasonably anticipated to be found:  (a)  29th Street ALF LLC, dba Meadowview of Greeley, at 5300 W 29th Street, Greeley, CO: On and preceding 11/13/19, during use, containers for contaminated sharps were not easily accessible to personnel or located as close as was feasible to the immediate area where sharps were used or could be reasonably anticipated to be found.  Employees assisted a resident with administration of insulin using a Lantus SoloSTAR insulin pen with Owen Mumford Unifine Pentip, and assisted a resident with blood sugar testing using ReliOn lancets.  Following use of the medical devices, employees transported the insulin pen with contaminated needle and lancet with contaminated needle from the resident's lower level apartment to the first floor med room.  The approximate one hundred foot path followed by an employee involved opening the resident's apartment door, walking to the stairwell or elevator, traveling upward to the first floor, walking down the hallway to the med room, unlocking the door, and entering the med room.  Once within the med room, employees removed by hand the contaminated needle from each insulin pen or lancet prior to disposal in the med room's sharps container. A sharps container was not located in the resident's apartment nor was a sharps container taken to the resident's apartment for immediate disposal of contaminated sharps.  This condition exposed employees to a bloodborne pathogens hazard.  (b)  29th Street ALF LLC, dba Meadowview of Greeley, at 5300 W 29th Street, Greeley, CO: On and preceding 11/13/19, during use, containers for contaminated sharps were not easily accessible to personnel or located as close as was feasible to the immediate area where sharps were used or could be reasonably anticipated to be found.  An employee administered an injection of vitamin B12 to a resident using a BD Safety Glide syringe with needle.  Following use of the medical device, the employee transported the syringe with contaminated needle from the resident's second floor apartment to the first floor med room.  The path followed by the employee involved opening the resident's apartment door, walking to the stairwell or elevator, traveling downward to the first floor, walking down the hallway to the med room, unlocking the door, and entering the med room.  Once within the med room, the employee disposed the contaminated sharp in the med room's sharps container. A sharps container was not located in the resident's apartment nor was a sharps container taken to the resident's apartment for immediate disposal of contaminated sharps.  This condition exposed the employee to a bloodborne pathogens hazard.

The employer's bloodborne pathogens training did not include training with regards to the facility's work practices and appropriate disinfectant to use during decontamination.  This condition exposed employees to a bloodborne pathogens hazard.    (c)  29th Street ALF LLC, dba Meadowview of Greeley, at 5300 W 29th Street, Greeley, CO: On and preceding 11/13/19, the employer did not ensure that the bloodborne pathogens training program contained an explanation of the use or limitations of methods that would prevent or reduce exposure including appropriate engineering controls, work practices or personal protective equipment. DAP Dynaflex spackle; and  13.  Sherwin Williams Corp ProMar 200 latex paint.

Employees cleaned blood spills or other potentially infectious materials spills.  Contaminated surfaces were cleaned with Ultra-Chem Inc Bio-Matic enzymatic cleaner.  The cleaner was not an appropriate disinfectant.  This condition potentially exposed employees to a bloodborne pathogens hazard.
