29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) Production Line 1 - On or about November 10, 2019, the employer failed to provide training to each employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. A Production Line 1 Pan Feeder sustained an amputation of the left ring fingertip while replacing a slipped chain on the chain lug pan conveyor.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Production Line 1 - On or about November 10, 2019, an employee was exposed to machine hazards associated with moving parts and in-going nip points during servicing of the Line 1 Chain Lug Conveyor, which involved replacing a slipped chain onto a moving sprocket. The employer failed to implement energy control application steps as the machine was not shut down or turn off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from servicing hazards.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Production Line 1 - On or about November 15, 2019, the employer failed to conduct a periodic inspection of the energy control procedure for the chain lug pan conveyor, at least annually, as required by the standard. Maintenance employees were required to perform service and/or maintenance tasks, and thereby exposed to hazards associated with the unexpected energization and/or startup of the equipment.
