29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about November 14, 2019 at 918 ML King Blvd., Macon, Georgia four (4) employees working on a 6/12 pitch roof installing were exposed to a fall of approximately twelve (12) to the lower when the personal fall protection was not used.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill. On or about November 14, 2019 at 918 ML King Blvd., Macon, Georgia employees working at installing a new roof were not protected from falling to the lower level. The employees indicated that they had been trained, but failed to utilize their fall protection. The employees failed to ascertain to the employers' fall protection policy or failed to understand the gravity of the hazard.
