29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Employees were exposed to amputation, crushing and abrasion hazards. The Machine Specific procedures for Lock Out Tag Out for the Shape B5, Acc-u-turn B5 were inadequate as the Standard Operating Instruction for Removing a Wheel Stuck on Punches S11-S15 does not refer back to the Shape B5, Acc-u-turn B5 Lock Out Tag Out Procedures. An employee was not required to utilize lockout tagout procedures when a wheel was stuck to the upper punch retainer tooling when the nitrogen was still turned on to the upper punch retainer tooling. On or about November 5, 2019, an employee received an amputation when the operator's finger got caught between the wheel and the S15 B Shape machine.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees were exposed to amputation hazards. The opening where the wheel exits the S15 B Shape machine is not guarded when the conveyor gate is lifted.
