29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: Employees were exposed to struck by hazards. Employees that drive the sweeper machines do not have any head protection when materials fall off of pallets from the racks. On or about November 12, 2019, an employee was injured when empty pallets fell from racking 20 to 30 feet above where he was working and struck him in the head.
