29 CFR 1910.252(a)(2)(vi)(C): Cutting or welding was permitted in the presence of explosive atmospheres (mixture of flammable gases, vapors, liquids, or dusts with air), or explosive atmospheres that could develop inside uncleaned or improperly prepared tanks or equipment which had previously contained such materials, or that could develop in areas with an accumulation:  Shop: Arc welding was performed on top of a drum of flammable window shield wiper fluid(contains methanol). Vapors were ignited leading to an explosive fire resulting in a impact and burn injuries.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):   Shop: A written hazard communication program was not developed and implemented where automotive maintenance and repair chemical products, including windshield wiper fluid(contains Methanol), were used.  A written program shall include information on how the requirement of labeling, safety data sheets and training will be implemented in the work place.
