29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Hardwood flooring manufacturing facility located at 560 Marine Dr. Calhoun, GA. 30701. On or about 11-15-2019, and at times prior, the employer exposed employees to amputation hazards in that the cutting blades on the two End Matcher machines located in the back left of the production area were changed out without the use of proper lock-out tag-out procedures.
