29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:  Employees exposed to struck-by and fall hazards.  At the jobsite located at 3009 N. Tee Time Wichita, KS:  Employees engaged in roofing activities were exposed to hazards such as struck-by and fall hazards while using pneumatic nailers, accessing and egressing roof using portable ladders not kept clear, and working from heights greater than 6 feet about the lower level without fall protection.  Employees were exposed in that the employer had not developed nor implemented safety and health programs that provide construction hazard recognition guidelines to mitigate those hazards.

At the jobsite located at 3009 N. Tee Time Wichita, KS:  Employees engaged in residential roofing activities using a pneumatic nailers and pitch forks were exposed to struck-by hazards in that eye protection was not provided.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Employees exposed to fall hazards.  At the jobsite located at 3009 N. Tee Time Wichita, KS: Employees engaged in residential roofing activities from a 8:12 pitch roof were exposed to fall hazards of approximately 9 feet 8 inches in the front and 13 feet 8 inches in the back from the eave of the residence to the ground below in that fall protection training was not utilized.

Tee Time Wichita, KS:  Employees engaged in residential roofing activities using portable ladders were exposed to fall hazards in that area around the bottom of the ladders was not kept clear in the following instances:  a) portable ladder in the front of the house next to the front entry  b) portable ladder in the front of the house next to the garage  c) portable ladder on the side of the house next to the air conditioner
