29 CFR 1926.20(b)(2):  Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:  On 11/18/2019, employees of Yoni Cruz performing roofing work at 7762 Quebec Court, OFallon, Missouri.  A competent person did not conduct frequent and regular inspections of the jobsite.  Employees on the site were exposed to hazards including but not limited to fall hazards and struck-by hazards.

29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On 11/18/2019, employees of Yoni Cruz were working on a residential building located at 7762 Quebec Court, OFallon, Missouri.  Employees were using a pneumatic nail guns without the use of eye protection, exposing them to a struck-by hazard.

The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards:  On 11/18/2019, employees of Yoni Cruz were working on a residential building located at 7762 Quebec Court, OFallon, Missouri.  The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.

29 CFR 1926.501(b)(13):  "Residential construction." Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 11/18/2019, employees of Yoni Cruz were working on a residential building located at 7762 Quebec Court, OFallon, Missouri.  Employees were installing shingles approximately 19 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The employees were observed on the roof of the building without fall protection.  Yoni Jose Cruz was previously citied for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1322204,  citation number 1, item number 1 and was affirmed as a final order on 7/2/2018, with respect to a workplace located at 109 Palm Drive, Saint Clair, IL.  Yoni Jose Cruz was previously citied for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1318052,  citation number 1, item number 1 and was affirmed as a final order on 7/2/2018, with respect to a workplace located at 3921 Gaslight Walk, Madison, IL.
