29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    On the east side of the construction site, employees engaged in framing activities used pneumatic nail guns and were not provided eye and face protection.  The employees were exposed to struck-by hazards and eye injuries.      Ruperto Olvera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1151408, citation number 01, item number 002 and was affirmed as a final order on 09/27/2016, with respect to a workplace located at 3332 Balcones Dr., Irving, Texas 75062.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).     On the east side of the construction site, employees engaged in framing work without fall protection, were exposed to fall hazards of approximately 11 feet to the lower level.        Ruperto Olvera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1151408, citation number 02, item number 001 and was affirmed as a final order on 09/27/2016, with respect to a workplace located at 3332 Balcones Dr., Irving, Texas 75062.  Ruperto Olvera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1065187, citation number 01, item number 002 and was affirmed as a final order on 09/10/2015, with respect to a workplace located at 3076 Denali Dr., Irving, Texas 75062.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.     On the east side of the construction site, employees using an extension cord to energize a Makita skill saw that had the outer sheathing pulled away exposing the internal wiring were exposed to electrical shock hazards.
