29 CFR  1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   The Employer did not provide the Occupational Safety and Health Administration (OSHA) certification of abatement as required for Citation #1, Items #1 & #2 issued December 11, 2019.  29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    Employee(s) were exposed to fall hazards. Employee(s) engaged in roofing activities were  exposed to a fall distance of approximately 12.5 ft from the roof to the ground below. Employee(s) were using ladders that was positioned at the edge of the roof and did not extend at least 3 ft above the upper landing surface.
