The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to caught-in hazards: a) At the facility, on or about November 16, 2019, employees worked at a conveyance system performing tasks such as, but not limited to cleaning around the conveyor that was activated without the knowledge of the employees.

1910.147(c)(7)(iii)(A) Retraining shall be provided for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures. a) At the facility, on or about November 16, 2019, the training for employees cleaning around conveyors did not ensure energy control procedures were understood by employees.
