29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. (a) On or about November 21, 2019, the safety interlocks were bypassed on the Mazak V-655/60 CNC machine ( Model No.MTV655/60 ) which exposed employees to an entanglement hazard. (b) On or about November 21, 2019, the safety interlocks were bypassed on the Mazak V-655/60 CNC machine (Model No.MTV655/60 and Serial No. 126340) which exposed employees to an entanglement hazard. (c) On or about November 21, 2019, the safety interlocks were bypassed on the Mazak Quick Turn 40 CNC machine ( Model No. QT 40 Serial No.138002 ) which exposed employees to an entanglement hazard. (d) On or about November 21, 2019, the safety interlocks were bypassed on the Mazak Quick Turn 40 CNC machine ( Model No. QT 40 Serial No.126097 ) which exposed employees to an entanglement hazard. (e) On or about November 21, 2019, the safety interlocks were bypassed on the Mazak Quick Turn 40 CNC machine ( Model No. MTV 414 Serial No.134512 ) which exposed employees to an entanglement hazard. (f) On or about November 21, 2019, the safety interlocks were bypassed on the Mazak machine center 2 CNC machine ( Model No. MTV 414 ) which exposed employees to an entanglement hazard (g) On or about November 21, 2019, the safety interlocks were bypassed on the Mori Seiki CNC machine ( Model No. SL-25 Serial No.1654 ) which exposed employees to an entanglement hazard (h) On or about November 21, 2019, the safety interlocks were bypassed on the Hyundai CNC Horizontal turning machine ( Model No. L300LC Serial No.0146 ) which exposed employees to an entanglement hazard. (I) On or about November 21, 2019, the safety interlocks were bypassed on the Hyundai CNC Horizontal turning machine ( Model No. L300LC Serial No.0147 ) which exposed employees to an entanglement hazard. (J) On or about November 21, 2019, the safety interlocks were bypassed on the Mazak Quick Turn 40 CNC machine ( Model No. MTV 414 Serial No.136095) which exposed employees to an entanglement hazard.
