Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly: (a) On or about November 13, 2019, 6-ft long fork extensions were used on the forks of the Hyster S35XL forklift truck without the manufacturer's approval. (b) On or about November 13, 2019, 6-ft long fork extensions were used on the forks of the Hyster S35XL forklift truck without changing the capacity and operation plate.
