One employee operates the TCT Automation TCT1 saw, Model: XL-Saw, S/N: XL13.11.16.211, in the production of wood trusses. The employee opens the covers to the machine and cleans the inside of the machine of wood dust without performing lockout/tagout. This condition exposed employees to laceration and amputation hazards.      b)    Builders FirstSource, Inc. at 4058 Camelot Circle, Longmont, Colorado 80504: On or about 11/19/2019 the employer did not ensure that procedures were utilized for the control of potentially hazardous energy.
