29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  A.  On or about November 20, 2019, Shop:  The emergency exit door, identified with an illuminated EXIT sign, at the North side of the building was locked during building occupancy.  The handle assembly had been knocked off and the door remained locked.

On or about November 20, 2019, Facility-Wide:  The employer had not certified that a workplace hazard assessment had been conducted to determine appropriate personal protective equipment to be worn by employees.   Employees working near powered industrial equipment were not required to wear protective footwear.

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel:   A.  On or about November 20, 2019, Shop, back corner near North exit door:  Electrical breaker panels were blocked by  the placement of a storage rack and compressor.  The panels were inaccessible in case of an electrical emergency, where the equipment needed to be turned off at the breaker.  The panels controlled 220V outlets, fans, lights and other outlets throughout the shop.

On or about November 20, 2019, Shop, Center Bay:  A red flexible extension cord was attached to the building surfaces, including to wall conduit, bay door track, overhead fluorescent light fixture and racking system. On or about November 20, 2019, Facility-Wide:  The employer had not developed a hazard communication program that described how labels and other forms of warning, safety data sheets, employee information and training would be met.  Employees used two-part chemical insulation in packaging materials and propane for the powered industrial vehicle.  Safety data sheets were not available and training had not been conducted.

29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent:  A.  Movealldotcom, LLC:  On or about November 20, 2019, the employer failed to maintain OSHA 300 Logs of Work-Related Injury and Illness in a year where work-related injuries meeting the general recording criteria were sustained.  On October 30, 2019, a driver helper was caught between a backing truck and a dock, resulting in a broken pelvis requiring days away from work.  Abatement Certification and Documentation is required.
