29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift.  (a)  On or about November 19, 2019, employees were working from a SkyJack SJ45T aerial lift replacing lights without the use of a safety harness and lanyard attached to the lift.  This exposed employees to a fall/thrown hazard of approximately 25 feet.
