29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure: a) Eco Brite Linens, LLC, Skokie, IL- On November 20, 2019, the employer did not make the hepatitis B vaccinations available to employees who were determined to have occupational exposure, such as the soiled linen sorters, or their supervisors, who find, handle and dispose of syringes/needles that have been reported to be found. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1904.29(b)(3): The employer did not enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. Eco Brite Linens, LLC, Skokie, IL- On November 20, 2019, the employer did not enter each recordable injury or illness on the OSHA Form 300, the Log of Work-Related Injuries and Illnesses, for calendar year 2019, within seven (7) calendar days of receiving information that a recordable injury or illness had occurred. a) An injury, contusion to both knees from a fall, that occurred on 9/11/19 and resulted in 41 restricted days was not entered on the 2019 OSHA 300 Log provided on November 20, 2019. b) A right arm strain from material handling that occurred on 9/30/19 and resulted in 26 restricted workdays, was not entered on the 2019 OSHA 300 Log provided on November 20, 2019. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
