29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: On or about November 14th, 2019, subcontractors were working without hardhats below a scaffolding that was 12 feet high while multiple other subcontractors were working on the top level of the scaffolding, exposing subcontractors to the hazard of being struck by falling objects.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below: On or about November 14th, 2019, subcontractors were working on scaffolding that was 12 feet above a lower level without a toeboard installed to protect subcontractors working below, exposing subcontractors to the hazard of being struck by falling objects.
