The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1 for employees exposed to fall hazards due to the ladder not extended at least three feet above the landing surface, and Citation 2 Items 1a/b for employees exposed to fall hazards and lack of fall protection and training.

On or about November 20, 2019, employees were exposed to a fall of approximately 10 feet while performing roofing activities on a residential home located at 21820 Norton, Bedford, Ohio 44146. The employees were using an extension ladder to access the roof and the employer did not ensure that it extended at least three feet above the landing area.
