29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1 for employees exposed to fall hazards due to the ladder not extended at least three feet above the landing surface, and Citation 2 Items 1a/b for employees exposed to fall hazards and lack of fall protection and training.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about November 20, 2019, employees were exposed to a fall of approximately 10 feet while performing roofing activities on a residential home located at 21820 Norton, Bedford, Ohio 44146.  The employees were using an extension ladder to access the roof and the employer did not ensure that it extended at least three feet above the landing area.

On or about November 20, 2019, employees were exposed to a fall of approximately 10 feet while performing roofing activities on a residential home located at 21820 Norton, Bedford, Ohio 44146.  The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.
